（中央社記者陳偉婷台北23日電）國內一追蹤超過20年的研究發現，C型肝炎感染者不只易有肝臟病變疾病，非肝臟器官罹癌風險也增加，且腦血管病變、腎臟病、糖尿病風險也偏高。
這項研究主持人為陽明大學臨床醫學研究所副教授李美璇，研究團隊西元1991年在台灣7個社區收案收了2萬多人，其中C肝感染者有1000多人，經過20年追蹤，並比對癌症登記檔和死亡狀況，得出研究結論。
李美璇今天受訪時表示，C肝病毒感染對肝癌、慢性肝臟疾病的死亡有關，但在長達20年的追蹤研究中發現，C肝病毒感染也跟非肝臟疾病有關，包含腦血管、腎臟病、糖尿病，也與非肝臟器官罹癌風險有關。
她說，C肝感染者相對於沒有感染者，有比較高肝外癌症風險，包含食道癌、前列腺癌、甲狀腺癌。以食道癌來說，C肝陽性感染者相對於陰性者，罹癌風險大4倍；前列腺癌也是4倍左右。
李美璇說，社區追蹤研究有排除癌症的危險因子，如食道癌好發於男性、抽菸喝酒者，但排除危險因子後，可看出C肝病毒和食道癌相關。
她說，過去美國也曾經有類似發現，但對致病機轉還不清楚，需要進一步基礎醫學研究才會知道，目前可以了解是病毒感染導致全身性發炎反應，發炎反應誘發免疫活化反應，比較容易導向心血管疾病、腎臟疾病方面的死亡。
李美璇說，感染C肝病毒後，有一部分人因免疫功能較好，可自發清除病毒，這類型感染者跟沒有感染過的人比較，罹癌風險差不多，但長期慢性感染者，肝外疾病的風險就會高，意即若有機會清除活化病毒，就能減少肝外疾病。
至於C肝和糖尿病關聯，李美璇說，兩者相關的證據已經確立，國外研究發現，C肝患者對干擾素治療有反應者，比起治療沒反應者，罹患糖尿病風險較低。1050623
